ORDER THE APPLICATION AND ACCREDITATION MANUAL

Once you determine that your agency will benefit from accreditation, you will need the following:

  • An Application for Accreditation
  • The Commission on Accreditation for Home Care (CAHC) Accreditation Manual

The CAHC Accreditation Manual contains detailed explanations of the Commission's Standards. You will need to have a thorough knowledge of the Standards to complete the application.

To request an Application for Accreditation and the CAHC Accreditation Manual, please print and complete the information form below and send it to the Commission along with:

  • A check for $300 made payable to “Commission on Accreditation for Home Care, Inc.” or “CAHC.
  • A copy of your agency’s current signed Health Care Service Firm license.
  • A completed Application and Accreditation Manual Order Form (PDF).

Workshops

If you have already received the CAHC Accreditation Manual or our Introductory
Information Packet
, you may be interested in attending a workshop. The
Commission periodically offers educational workshops which give you the
opportunity to increase your understanding of the Standards, ask questions,
and learn how to incorporate the Standards into your operations.
